The Royal Marsden NHS Foundation Trust is holding its 9th Annual Breast Cancer Meeting on Friday 7th October 2016. As in previous years, we aim to provide an update on what is new, challenging and controversial in the management of patients with breast cancer. This year we will address some dilemmas during neo-adjuvant therapy including imaging techniques and surgical management of the axilla. We will discuss some of the common questions that patients ask, namely who needs genetic testing nowadays and what diet should be followed, in addition to providing an update on molecular testing and emerging novel therapies.

Disorders in lipid metabolism have now been implicated in the development of many diseases including cancer and cardiovascular diseases. At this stage, the metabolism and function of lipids are poorly understood in part due to the complexities of lipid metabolism regulation. While the role of cholesterol and other lipids has been evident for many years in cardiovascular diseases, their role in cancer development and progression is now beginning to be elucidated. Interestingly, drugs that are used to treat lipid metabolic disorders may show their usefulness for the treatment of cancer. Importantly, new research in drug development suggests that lipid metabolism may be used as a target for the treatment of cancer. This new approach will include the use of biopharmaceuticals and nanotechnologies.

Metastatic disease, the lethal component of cancer, remains largely intractable to modern medicine. New insights into the biology of metastasis promise fresh insights into how to design therapies that effectively tackle this life-threatening disease process. Recent years have seen the development of novel tools for studying metastasis that continue to provide dramatic advances in our understanding of the cellular and molecular mechanisms that drive metastasis formation. Metastasis is regulated by both tumor cell-intrinisic factors such as genetic defects, epigenetic perturbation and metabolic changes, as well as by a variety of factors in the microenvironment, including cells of the immune system. These tumor cell-intrinsic factors combine with the external microenvironment to regulate tumor dormancy and the development of therapy resistance. This knowledge is starting to be translated into therapies that target not only the metastatic seed but also the microenvironmental soil.

Lately the field of epigenetics has been rapidly changing and progressing. Advances that were made possible because of the Human Genome Project have opened up new possibilities about how epigenetics can be used to stop cancer. Scientists have recently discovered that there are potentially two sets of enzymes that are responsible for regulating the chemical structure of chromatin, which consists of the methyl and acetyl marks in the chromatin structure. Theoretically, by changing and controlling the chromatin structure, scientists can attempt to gain epigenetic control of DNA in order to potentially stop aberrant gene expression.

This clinical experience programme provides participants with an overview of the spectrum of malignant disease. The manifestation, presentation, investigation and management of a range of common solid tumour types (breast, lung, colorectal, gastrointestinal, melanoma, renal,) are reviewed. Clinicians active in these areas examine patient management throughout the process. Imaging techniques and radiotherapy treatment are explained and these departments visited. The context of cancer care is also reviewed, including the epidemiology of cancer and factors affecting treatment choices and prescribers' decision-making. This clinical experience programme combines taught lectures and clinical visits, providing a unique opportunity for delegates to experience, first hand, the assessment and treatment options utilised within the overall management of patients with malignant disease. During the last decade, it has become unequivocally evident that tumor development is not a cell-autonomous process but rather depends on the intricate reciprocal interplay of mutant tumor cells with their local and distant environments. Composition and polarization of cells in the tumor microenvironment depends on genetic as well as environmental factors and is directly influenced by tumor therapy. Indeed, therapies that aim to shape the local immune milieu and consecutive signaling pathways in both stromal and tumor/stem cells address the complex pathophysiology of tumors more adequately and may therefore add substantial benefits for therapy. An absolute pre-requisite for such an endeavor is a comprehensive understanding of the exact molecular basis of the complex signaling networks in the tumor microenvironment that control the plasticity of both stromal and tumor cells, thereby shaping the complex cellular contexture, which ultimately forms a pro- or anti-tumorigenic milieu. Thus, this symposium aims to gather a comprehensive functional understanding of mediator-dependent cellular and molecular events that are responsible for the plasticity of both stromal and tumor/stem cells. It will bring together experts in cancer, stromal and immune cells to synthesize scientific knowledge about the phenomenon of cell plasticity within the tumor microenvironment, to define molecular and cellular pathways mediating plasticity and to propose approaches to interfere with cell plasticity for a new generation of effective therapeutic approaches in cancer and chronic injury. TGF-beta is a key regulator of immune responses, inflammation and cancer. In addition to the well-established effects of TGF-beta on CD4+ T cell differentiation and reprogramming, TGF-beta also plays critical roles in other immune cells. TGF-beta has also been implicated in the regulation of the interactions among immune cells, microbiota and epithelial cells. In addition to safeguarding immune homeostasis and suppressing chronic inflammation and autoimmunity, TGF-beta is emerging as a potential checkpoint in anti-tumor immunity. Despite great progress in the field, key questions related to TGF-beta functions in immune responses are still largely undetermined. Therefore, it is imperative to understand the cellular and molecular mechanisms by which TGF-beta controls such diverse immune cells. Elucidation of these pathways would allow us to understand the pathogenesis of chronic inflammation, autoimmunity and cancer and aid development of therapeutic practices in experimental and ultimately clinical settings. This meeting will gather together experts in the fields of both TGF-beta signaling and immunology to discuss new findings and update the pivotal role of TGF-beta in immunity, inflammation and cancer, with the aim to lay out the critical and key questions and outline new directions.

It has become apparent over the last decade that small and long noncoding RNAs play a major role in the regulation of gene expression and disease. While much emphasis has been centered on the role of noncoding RNAs in human disease, a gap remains in utilizing this knowledge to develop target specific approaches to disrupting noncoding RNA pathways for therapeutic gain. This meeting will for the first time bring together an interdisciplinary group of basic and applied noncoding RNA scientists in an effort to translate our current understanding of noncoding RNA biology into meaningful therapeutics for many of those diseases afflicting society. To accomplish this goal, the meeting will aim to: (1) Introduce the broad genome-wide basic fabric of noncoding RNA regulation and form and function in the cell; (2) Focus on direct RNA/chromatin/DNA or RNA/protein interactions with candidate disease-relevant model systems; and (3) Highlight synthetic biological approaches to developing RNA or oligonucleotide mimics to target and/or disrupt particular disease-relevant noncoding RNA pathways. The goal of this meeting is to bring together basic and applied scientists working on noncoding RNAs in an effort to not only better understand the role noncoding RNAs play in the regulation of disease-relevant genes but also the emerging ability to utilize this knowledge to develop targeted therapeutics, an eventuality that heralds a new frontier in drug development and therapeutics.

Defect in DNA damage repair and checkpoint control is the underlying mechanism for tumorigenesis, since it allows the accumulation of multiple genetic alternations, which are essential for the initiation of tumorigenesis. This has been clearly illustrated to be the cause of several human cancer-prone syndromes and also revealed by recent human genome studies. On the other hand, defective DNA repair and checkpoint activation also make cancer cells more vulnerable for particular DNA damaging agents or inhibitors that specifically disrupt some of these checkpoint pathways. With the increasing understanding of defects in DNA repair and checkpoint control in tumorigenesis, there are extensive interests in exploring these deficiencies, especially taking advantage of the synthetic lethality concept and targeting particular DNA repair and checkpoint pathways for cancer therapy. The purpose of this conference is to bring together basic, translational and clinical investigators and discuss the current and future directions, opportunities and obstacles in the development of these anti-cancer modalities and how to best apply these concepts in clinical practice.

Cellular responses to changes in oxygen availability have important implications for both physiology and pathophysiology, in particular in the setting of solid tumor progression. A number of hypoxia-inducible pathways have been characterized, such as the large transcriptome regulated by hypoxia-inducible factors (HIFs). However, many other oxygen-sensing pathways exist within the cell. The aim of this meeting is to disseminate up-to-date information on basic mechanisms of cellular and tissue hypoxia responses, and discuss the impact of these responses on physiology and cancer, with an emphasis on HIF-independent, but nevertheless fundamental, oxygen-sensing pathways. Outcomes of this meeting will include the dissemination of new scientific knowledge, the discussion of new methodologies, the education of graduate students and postdoctoral fellows in this discipline, and promotion of new collaborations between clinicians, basic researchers and industrial partners. This will be the eighth Keystone Symposia meeting on Hypoxia, which brings together academic, clinical and industrial researchers to enhance synergy between these groups and develop translational strategies for targeting hypoxic stress responses for therapeutic benefit.

In advanced prostate cancer, rapid and successful drug development has resulted in multiple treatment options, requiring careful decision making for individual patients. Clinicians face the increasingly difficult task of choosing from multiple potentially effective treatments that are also costly and potentially toxic. The inaugural Advanced Prostate Cancer Consensus Conference (APCCC) did provide a forum for discussion and debates on current treatment options for men with advanced prostate cancer. The conference concluded with voting by the panel on pre-defined consensus questions, targeting areas of primary clinical relevance. Following the conference a manuscript with containing the expert recommendations was published at Annals of Oncology.

While the importance of adoptive and innate immunity has been clear in the pathogenesis of human autoimmune disease resulting in a multitude of immune-based therapeutic approaches, the realization is now apparent that understanding immune evasion by cancer is central in developing curative treatments. This meeting will explore and contrast the underlying immune mechanisms resulting in autoimmunity and tumor evasion. The meeting is innovative in bringing together basic immunologists investigating mechanisms of tolerance with scientists exploring immune mechanisms of autoimmunity and cancer in both patients and experimental models. Thus, this Keystone Symposia meeting will cover the pathways in immunity and tolerance that lead to loss of immunological control, dysregulated immune responses and chronic inflammatory disease or tumor evasion. Presentations will include consideration of preclinical and clinical aspects of a diverse number of autoimmune and inflammatory diseases and cancer. Conference participants engaged in preclinical, translational and clinical research will hopefully engage in continuing conversations and collaborations which, over the long-term, will provide greater insights into the human immune response and allow us to reassess and further explore pathways that are driving autoimmune disease yet in opposition, lead to tumor evasion. Understanding checkpoints in autoimmunity and immune cell tolerance is important for delivering therapies to patients with autoimmune disease and cancer, and this meeting will provide a platform for the cross-pollination of clinical experience and experimental research. Attendees will have learned about the impact of targeted immune-based therapeutics on clinical outcome and, consequently, be able to widen their research scope accordingly.

Genome stability is the foundation upon which all cellular and organismal processes depend. This conference will grapple with the intricate array of biochemical reactions orchestrated by the cell to replicate, repair and segregate chromosomes accurately despite constant threats from spontaneous and environmentally-induced damage. Defects in these reactions lead to genomic instability, a confirmed driver of cancer and degenerative diseases. Fascinating and therapeutically crucial questions remain about the mechanisms underlying chromosome stability, and cross-disciplinary approaches are required to address them. How do epigenetic chromatin marks influence repair of underlying DNA sequences, and how can we predict the effects of chemotherapeutic agents that alter such marks? To what extent is the nucleus organized into subdomains with distinct functions; how are these subdomains altered when cells move or differentiate? How are the myriad helicases, nucleases and polymerases coordinated to safeguard genome stability, why are these molecules often mutated in human disease, and how can we ameliorate the effects of such mutations? What is the array of strategies available to cancer cells as they achieve unlimited proliferation? This conference has a history of bringing together investigators from diverse subfields who otherwise rarely meet. Cutting-edge concepts in translational, genomic, cellular, molecular, RNA and structural biology will be dissected to take the conversation to unprecedented levels of depth and breadth. The concurrent conference on “DNA Replication and Recombination” will provide additional opportunities for cross-talk. Both conferences are committed to nurturing interactions among longtime experts in the field with students, postdocs and investigators new to the field.

The International Workshop on Chronic Lymphocytic Leukemia (iwCLL) is a biannual meeting focused solely on advancing the understanding and treatment of chronic lymphocytic leukemia (CLL) and related lymphoproliferative disorders. Founded in 1979, iwCLL is now the largest international meeting dedicated to these blood cancers, leading the scientific discussion on both laboratory-based research and clinical practice. The biannual meeting provides a forum that facilitates the exchange of information and encourages collaboration among international leaders and scientists interested in CLL and other lymphoproliferative disorders.

The maintenance of genome integrity is critical for the suppression of cancer and premature ageing. Only recently has it become appreciated that DNA replication stress is a crucial driver of genomic instability. The timely progression of replisomes can be disrupted by lesions and secondary structures in the template, by bound proteins and by conflicts with the transcription machinery. A prolonged pause of the replisome then exposes single stranded DNA, which, due to its recombinogenic nature, can lead to genome rearrangements, fragile site expression and cell death. Importantly, some cancers present excessive endogenous levels of replication stress, which can be exploited for their clearance. In this conference, we aim to bring together scientists studying DNA replication and repair, with those interested in how DNA damage can influence cancer and ageing.

This conference will provide an opportunity to learn about and keep up to date with the rapidly progressing area of cancer genomics. It will cover presentations from cancer genome projects, the areas of cancer functional genomics, systems biology, cancer mouse models and the translation and clinical impact of scientific results obtained. The meeting will bring together leading scientists from across these areas for a unique opportunity to interact and stimulate further integration of these efforts.

European Cancer Genomics research is particularly strong in the Heidelberg area, with three International Cancer Genome Consortium (www.icgc.org) projects being currently pursued with significant participation from Heidelberg institutions, including EMBL. As in our 2015 meeting, a large number of renowned speakers will be invited, with a strong representation of overseas speakers. The atmosphere at the conference will make this an exceptional event with cutting edge science, extensive scientific discussions and ample possibilities for meetings between and after sessions to facilitate in-depth discussions and new collaborations.

The Indian Cancer congress is an amalgamation of the four major national oncology associations ( The Association of Radiation Oncologists of India, Indian Association of Surgical Oncology, the Indian Society of Medical & Pediatric Oncologists and the Indian Society of Oncology) . This conference is a planned quadrennial event of these four associations and all other sub specialty associations to improve the quality of cancer care through multidisciplinary interactionand will be held in various parts of the Indian subcontinent once every four years. The ICC2017 conference is scheduled to be held from 9th to 12th November 2017 (with pre-conference workshop held on 8th Nov 2017) in Bengaluru, India.The theme for the conference is “INSIGHT, INNOVATION, INTEGRATION” The field of oncology is rapidly evolving and is in constant flux. What is thought to be true today may very soon be turned on its head by new insights and new innovations. It is therefore prudent and necessary that we the oncology community – periodically stop in our fast tracks and take stock of some important insights and innovations and debate how best we can integrate these innovations into our practice. The congress hopes to attract each and everyone involved in the care of the cancer patient including physicians, basic scientists, patients, survivors , care givers, researchers, investors etc and to provide a platform from which meaningful value addition can emerge.

The Cancer Imaging and Therapy conference will focus on optical methods in cancer, centered on imaging and treatment of cancer. The conference will accept submissions on topics on the molecular and cellular level, such as reporters and theranostics; on the tissue and pre-clinical (animal) level, such as pathology methods and advances in imaging of tumor models; and on human tissue and living human subjects, when specifically dealing with cancer. More generic biomedical optics and biophotonics research involving human tissues or living human subjects should be submitted to the Clinical and Translational Biophotonics conference at this congress.
